DEIAO Committee Volunteer Intake Form

Thank you for your interest in applying as a Volunteer to the Diversity, Equity, Inclusion and Anti-Oppression (DEIAO) Committee of the CES-ON.

The information you will share with us will be kept confidential and private. Your information will be kept in a password-protected file in the CES-ON Dropbox’s folder and only the Co-Chairs of the DEIAO Committee, the President of CES-ON, and a person appointed by the Committee to store and/or conduct the analysis of the data will be able to see that you participated in applying.

To help us identify the diverse reach of this volunteer recruitment process, data from this form may be used for internal DEIAO Committee analysis. In this event, we will delink participants’ names from data results. Findings will be presented only in summary form and your name would not be used in any report. While the DEIAO Committee will keep your information confidential, there are some risks of data breaches when sending information over the internet that are beyond our control.
